The sealing of a tooth root-end post apicoectomy is somewhat problematic and such endodontic treatment can fail. This is usually the result of leakage of the material used in the retrograde filling. Microleakage has been associated with amalgam, zinc oxide eugenol cement and glass-ionomer cement. Leakage from mineral trioxide aggregate has been shown in some studies to be less prone to leakage than these other materials but there is evidence that leakage may increase over time.

Gingival recession and/or root sensitivity, and aesthetic concerns involving the anterior or bicuspid teeth are conditions that may require soft tissue management via tissue grafting. Potential problems associated with the placement of mandibular implants in the anterior jaw region between the mental foramina include dehiscences or bone fenestrations, mandibular fractures, injuries to the mental nerve, and perforation of the lingual cortical bone during surgery with associated hemorrhage into the floor of the mouth. Thus, prior to surgery, it is important for the surgeon to have accurate x-ray knowledge of bone quality, ridge dimensions such as bone height and width, the location of the mental foramen, mental loop and incisive canal, and possible bone pathology.
